An international initiative to develop an innovative computerized diagnostic system for the prognostic monitoring of systemic and renal hemodynamics and prevention of pre-renal acute kidney injury.
Pathophysiology, pharmacokinetics, pharmacogenomics, systems medicine, mathematical modelling and software engineering.
Drugs, medical and diagnostic procedures may substantially alter systemic and renal hemodynamics leading to pre-renal acute kidney injury (AKI) and other syndromes, including cerebral hypoperfusion. Co-morbidities and predisposing factors, including environmental and genetic factors, determine the hemodynamic effect of drugs and procedures, as they set the pathophysiological grounds on which all these factors act. In order to gain a holistic view of hemodynamics, more complete pathophysiological knowledge on many different circumstances combining factors potentially altering systemic and renal hemodynamics, BP, volemia, RBF and GFR must be gained from empirical experimentation. This knowledge, along with pharmacokinetic profiling, and genetic and pharmacogenetic knowledge on inter-individual differences in drug handling, distribution, elimination and metabolism must be integrated through a systems medicine approach with the molecular signalling networks involved. Integrated knowledge should then form the basis of a mathematical modelling system coupled to friendly software interfaces that allow professionals to monitor the patient’s hemodynamic profile in a personalized manner; and to prevent undesired outcomes by predictive forecasts provided by the model when challenged with additional factors or manoeuvers. . We intend to lead a consortium and a H2020 proposal to fully develop a first operating version of this integrative diagnostic system. For this purpose, partnering and cooperation is sought from industrial and academic entities with different pre-clinical, clinical and technological capabilities.
